Per Diem Health Educator - Requires Graduate from an accredited nursing program.? Must have Current NJ RN. PA RN required to obtain within 60 days of hire.? Certified Diabetes Educator (CDE) or Board Certification in Advance Diabetes Management (BC-ADM) with at least 2 experience as diabetes educator preferred; insulin pump certifications preferred; valid driver's license. Function as Diabetes Educator for ADA recognized education program; Develops tools and conducts a thorough, individualized needs assessment with patient, significant other participation, as appropriate and outlined in the current standards of Practice for Diabetes Education. Provides programming for diabetes consistent with national standards for diabetes patient education programs as established by the National Diabetes Advisory board. Develops a written educational plan based on patient's educational/health assessment, using all components of the educational progress. Develops mutual goals with patient, describes these behaviorally and evaluates and revises the goals on an ongoing basis. Provides individual education which progresses from basic survival skill education to advanced information necessary for daily self-management. The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
